IDS Registration Form (2017-18)

All new and returning students must please fill out the 2017-18 registration form below to register for the new season. Please use the current 2017-2018 schedule to choose the appropriate classes for your dancer, with careful attention to age and experience level. If you would like a copy of this page, please print before submitting. Once the form has been submitted, all payments must be made at Imperial Dance Studio. Imperial Dance Studio Policies *
• THERE ARE N0 refunds for classes missed, however, any class can be made up un an alternate class within that month. • PARTICIPATION IN THE RECITAL and/or competitions is optional. Students are welcome to attend classes all season long, regardless of whether or not they choose to participate in the show. • COSTUME PRICES ARE $75.00 (per costume, per class) for ages 6 and under, and $95.00 (per costume, per class) for ages 7 and up. • RECITAL QUALIFICATIONS: Each performer is required to purchase a minimum of 5 tickets per family. TICKETS ARE $25.00 each. • ALL BALANCES must be paid in full including the full month of July tuition in order to participate in the July dance recital. • A ONE TIME Registration Fee of $25.00 (for new students) will be added along with your first tuition payment. • WE ACCEPT cash or check only. • THERE WILL BE A $20.00 service fee for any returned check.
